Each week, Ƶ is highlighting how our platform supports patients and care teams for specific surgical procedures or health conditions.

Our solution uses a patient’s own smartphone, tablet, or desktop computer to guide them with health system-specific, step-by-step guidance via reminders, education and symptom monitoring.

This week’s spotlight: Post-Myocardial Infarction (Heart Attack) Care.

Highlights from the Post-Myocardial Infarction Care Journey

Personalized Recovery Setup

  • Patients begin with a tailored setup survey to capture medical history (e.g., diabetes, prior cardiac implants, physical limitations).
  • Questions dynamically adapt the program – for example, patients with mobility challenges won’t be shown walking-based tasks.
  • Visual aids help patients understand treatment-related terms, such as stents.

Daily Health Check-Ins & Symptom Monitoring

  • Patients complete daily surveys to track common warning signs after a MI and related treatment - symptoms like chest pain, mood, and wound healing.
  • Smart alerts provide urgent recommendations (e.g., seek emergency care for chest pain or shortness of breath).
  • If they had a recent cardiac procedure, patients can upload wound photos to share with their care team, supporting early detection of complications.

Medication Support & Side Effect Management

  • Patients receive guidance on taking their prescribed medications correctly and consistently.
  • The program educates on side effects (e.g., ACE inhibitor–related cough) and provides recommendations such as contacting a pharmacist or requesting blister packs for better adherence.
  • Dietary support is built in, including prompts about sodium and potassium restrictions.

Education & Lifestyle Guidance

  • An Educational Library offers resources on life after a heart attack, including nutrition, emotional support, and safe activity levels.
  • Video content from trusted organizations like the American Heart Association reinforces best practices for recovery.
  • Practical “Learn Why / Learn How” tools help patients understand not just what to do, but why it matters.

Ongoing Progress Tracking

  • Patients can visualize trends in blood pressure, mood, and other vital indicators over time.
  • This tracking fosters engagement and enables more informed conversations with caregivers and providers.

Why it matters

Recovering from a heart attack can feel overwhelming for patients who suddenly face new medications, lifestyle changes, and risks of complications. Ƶ’s Post-MI Care Journey provides step-by-step digital guidance so patients never feel alone in their recovery. It empowers them to recognize warning signs early, stay adherent to medications, and build lasting heart-healthy habits.

For care teams, Ƶ reduces preventable readmissions, improves patient engagement, and ensures providers can deliver proactive, personalized support at scale.
